Service requests or complaints may be submitted on-line or printed and then faxed or e-mailed.
If you prefer, you may also print a form to complete off-line: Printable Service Request/Complaint Form, and then e-mail to email@example.com or firstname.lastname@example.org, or fax it to the Highway Department 845-758-4617.
